We do accept most forms of medical insurance with the exception of Medicare/Medicaid. Reimbursement is based on the benefits of your individual plan and we will be able to notify you of those benefits once your insurance has been verified. You may also use pre-tax dollars from a Health Savings Account (HSA) to pay for your physical therapy services. To bill insurance for physical therapy, you must have a doctors referral.
Do I need a referral from my doctor?
If you are seeking physical therapy for a particular problem, you are required to have a referral from your physician, chiropractor, or nurse practitioner. If you are asymptomatic and are looking for a way to keep your body healthy and fit, we can train you without a referral.

What do I wear?
Wear anything you can move in freely. Shoes and socks will be removed during Pilates sessions; however, you may wear socks with grips.
What is your cancellation policy?
Except for emergency situations, you must cancel within 24 hours of your scheduled appointment time or you will be required to pay the full price of your treatment session.
Do you only perform physical therapy using Pilates?
No. Re-form Physical Therapy & Pilates is a full service physical therapy clinic and can treat anyone requiring physical therapy services. Our physical therapist is certified in vestibular dysfunction and vertigo rehab as well as LSVT Big and Loud for Parkinson's. Most musculoskeletal and nueromuscular disorders will be treated using Pilates techniques and equipment and will be tailored specifically to meet your needs.
