Jentry Bradley, PT, DPT
Owner/Founder

Jentry is the owner and founder of Re-Form.  She graduated from Spearman High School in 2004, and earned her Doctorate of Physical Therapy from Texas Tech University Health Sciences Center in 2011 following completion of her BGS with concentrations in Psychology, Business, and Biology from Texas Tech University in 2008.  After working in several specialty areas, Jentry founded Re-form in 2015.  She discovered her passion for Pilates while rehabilitating from a hip injury in college and knew someday she would integrate it into her physical therapy practice.  Jentry is a fully certified STOTT Pilates instructor and is currently working toward her certification in the STOTT Pilates Rehab Program.  Jentry's areas of expertise include spinal cord injury rehab, neurological disorders, and movement and muscle recruitment disorders. She is also a Certified Wound Therapist through the International Lymphedema and Wound Training Institute. Jentry is a community advocate and currently serves on the Turn Center Medical Advisory Board, the Amarillo College Foundation Board, the TTUHSC DPT Clinical Education Advisory Committee, and the St. Andrews Episcopal School Board.

Jentry's vision for Re-form is to offer a calming and friendly environment where clients and patients will receive the utmost level of respect and support to achieve their goals while receiving the highest quality service and skill.​

Erin Park, PT, DPT

Clinic Director for Physical Therapy and Wellness

Erin earned a Doctorate of Physical Therapy from Texas Tech University health Sciences Center in 2013.  She also holds a certificate in Women's Health Studies from Texas Women's University and has personal interest in treating pregnancy and postpartum pain.  Erin is extremely passionate about taking care of patients with conditions ranging from orthopedic injuries to vestibular conditions.  

Erin started training with STOTT Pilates in 2021 and has completed the matwork, reformer, cadillac, and chair coursework.​

Lillian Lopez, PTA
STOTT Pilates Education Coordinator

Lilly is a 2016 graduate of Amarillo College's Physical Therapy Assistant program.  She works closely with the physical therapists at Re-form and has extensive training in outpatient orthopedics as well as pelvic floor rehabilitation.  Lilly started training with STOTT Pilates in 2021 and has completed the matwork, reformer, cadillac, and chair coursework.   She is also currently working on her Bachelor of Biology Degree at WTAMU.

Lilly is highly compassionate and enjoys working with others. She strives for success, and always aspires to gain knowledge.  Lilly is focused on her patients overall quality of life and seeks to improve  mobility and functional performance in all of her patients.  Lilly is bilingual and proud to offer quality and consistent care to Spanish speaking patients.

Dianna Fuller, PTA

Physical Therapist Assistant

Diana Fuller has been active in the field of physical therapy since graduating from Caprock High School in 2012, where she initially worked as a technician. She completed Amarillo College's Physical Therapy Assistant program in 2023 and holds a Bachelor’s degree in Biology from West Texas A&M. Fluent in Spanish, she finds joy in witnessing the significant yet subtle achievements that patients share or demonstrate during their sessions.​

What she appreciates most about Re-form is the inclusive atmosphere and the comprehensive approach to a patient's overall well-being. Diana is particularly inspired by the small victories that have a profound impact on patients' lives, such as regaining the ability to reach overhead to fix their hair, returning to running, or enjoying quality time with their grandchildren.

Tifany Ingram
Front Office Manager

Tiffany, who serves as the Front Office Manager at Re-Form, brings over 25 years of experience as a medical assistant and a certified phlebotomist. With extensive office experience, she cherishes the warm and positive atmosphere that resembles a close-knit family, from colleagues to clients. Tiffany appreciates Re-Form's diverse services tailored to meet individual needs.

Nikkia Caple
Business Administrator/Compliance Office

Nikkia Caple brings a wealth of experience and passion to her role as the Business Administrator at Re-Form. With a background as a physical therapist technician spanning seven years, Nikkia has honed her skills in providing exceptional care and support to clients. During that time, she earned bachelor’s degree in biology from West Texas A&M University. Among her many duties at Re-form, Nikkia handles new therapy patient scheduling, and therapy billing and insurance.

Nikkia cherishes the friendly and positive atmosphere created by both the employees and clients at Re-Form, finding joy in the collaborative and uplifting environment. To her, the definition of “Re-form" – to make changes in something in order to improve it – is a guiding principle. She is dedicated to the continuous growth and improvement of both the organization and its clients, believing that everyone at Re-Form is constantly evolving towards their best selves.

Mandy Godsey is a receptionist dedicated to creating a welcoming and professional environment for every guest and team member. With a strong background in customer service and the ability to manage multiple priorities, she handles every interaction with care, efficiency, and attention to detail.

As a mother of four, Mandy understands the importance of organization, clear communication, and making a positive first impression. Whether assisting with scheduling, answering questions, or supporting the team behind the scenes, she is committed to providing exceptional service and ensuring the office runs smoothly.
