1. Do I need a physician referral?
In the state of Texas, a physical therapist can legally evaluate and treat patients within their scope of practice for 10 business days without a physician referral. Re-Form does offer direct access and accepts patients without physician referral, however we do understand the value of an interdisciplinary approach and will communicate with your physician or healthcare provider regardless of the duration of your therapy needs.
2. Do you accept insurance & self pay?
We do accept insurance and are currently in network with most major insurances companies and Medicare.

- Initial Evaluation - $140
- Follow-up visits - $120/session
3. Do you offer physical therapy fitness programs?
Suppose you are a Pilates or fitness client of Re-form and need a consultation with a physical therapist. We are available to provide consultation and will communicate with your Pilates or fitness instructor to modify your program, provide physical therapy services if indicated, or communicate with/refer you to a physician if needed.
Some clients prefer to have a physical therapist as their regular Pilates or fitness instructor due to certain conditions they are managing. We call this Clinical Pilates/Fitness. Whatever your needs, we will make sure to get you appropriate care and instruction, even if it isn't with us!

What to Expect & Getting Started

At your first visit, your therapist will perform an in-depth, one hour evaluation to determine your specific needs and impairments.
This begins with a subjective history assessment — including describing the onset and nature of your symptoms, pain level and patterns, and reviewing pertinent medical history and previous treatment. We will then perform a thorough physical exam to determine the cause of your symptoms. This may include evaluating your range of motion (ROM), strength, flexibility, aggravating movements, and special tests. Based on the information gathered in the assessment, your therapist with discuss a treatment plan that meets your needs physically and financially. |
